Steps to Completing the Assignment • Create a Data Dictionary: A data dictionary provides details about specific items in a database. In the LaTour book, there are several pages on data dictionaries. The information for your data dictionary is in the MedPAR pdf. Look for the file link in the assignment. You need to access this information to find the answers to the questions. Standard information includes: o Table name o Attributes or field names o A description of each attribute/field name o Data type (text, number, data, etc.) • Vlookup is a function in Excel that allows the user to combine records from two worksheets. o Add a column for gender description in the first tab of the CHF dataset. Type “Gender Description” into cell M1 (size and wrap text the heading cell) o Populate that column using vlookup o Add definitions to gender (sex) on the Data Dictionary tab of CHF Data Spreadsheet o On Data tab worksheet, Type this formula into cell o M2: =VLOOKUP(D2,'Data Dictionary'!C$5:D$7,2,FALSE) o The arguments for vlookup are: o What to look up (value in D2). (lookup value) o Where to look it up (cells C$5 to D$7 on the Data Dictionary tab) – note that the ‘$’ appears between the row and column designation for the cells holding the definitions. Note•I am taking a slightly different approach to showing you how to do the VLOOKUP•It does not match exactly what is in the instructor guidance, but I am hoping this is easier.•Part of the problem might be what version of Excel you have on your computer.
Step 1: The two files•The first file you need is the CHFdataset–The CHFdatasetcomes from the MedPARdataset. Step 2: The Data Dictionary•I think this is where some of you are getting lost.•When you open the second file, you will see a document that defines each column in the dataset. You need to use the values in this file for the VLOOKUP.
Step 2: The Data Dictionary•In the CHF dataset file that you downloaded from the course, you want to go to the third worksheet.–The first worksheet is Data–The second worksheet is Data Dictionary–Click on the next one that says Sheet
Example: Adding the Gender•We will do one together and then you just need to repeat the process for the other data elements.–Use the second file that documents the dataset–Look at page five and six of the document and you will see where the definitions are located•Age is on page 2 (With a pdffile, you have to scroll down a little since it is not always matching the page number. For example, the age breakdown is on page 11; the gender breakdown is on page 12; etc.)

Step 3: Adding the column to the data file•What is VLOOKUP?–Think of it as a translator.–On the data file worksheet, you see Sex but the values are 1 or 2 and you do not know what that means.•On the Data worksheet, go to column M1and enter Gender Description•You have two options now.•Now we will do the translation
Step 3: Look up•Option 1: Type in =LOOKUP(D2,Sheet1!A$3:B$5)–It has to be exact–This is similar to how the instructor guidance is worded.•Option 2: Use the Excel function
Step 3: Translation•With your cursor in M1, select insertfrom the Excel ribbon (The blue line )–Home InsertPage Layout Formulas Data Review etc.–Now click on fx(function)–Select All–You are going to scroll to find the LOOKUP function. It might say VLOOKUPor it might just say LOOKUP–Click on LOOKUP–Hit OK–It should ask you which one you want and choose lookup_value,array–Select OK
Step 3:•Lookup_value[This is your starting point. I want Excel to take the value in D2and find the matching description] data worksheet–Click on the symbol of an excel file, and click on D1•Table_array[This is the range in your lookup table]–Click on the symbol of an excel file, and click on the sheet and highlight A3to B5•Select OK
Step 3:•When you look at M2, you should see Female and if you look at the content of the cell it will look like =LOOKUP(D3,Sheet1!A3:B5)•Now we want to prepare the contents to be copied to the remainder of the row. Insert the $ sign after A and after B and it will look like•=LOOKUP(D2,Sheet1!A$3:B$5)
Step 4: Complete the column•Click on M2•In the lower right hand of the M2box, click on the box until you see the + sign•Now dragto the last row with data•You should now see Column M filled in with Male or Female

EXPANDED MODIFIED MEDPAR FILE The Medicare Provider Analysis and Review (MEDPAR) file contains records for 100 percent of Medicare beneficiaries using hospital inpatient services. The records are stripped of most data elements that will permit identification of beneficiaries. The hospital is identified by the six-position Medicare bill number. The file is available to persons qualifying under the terms of the routine use act as outlined in the December 24, 1984, Federal Register, and amended by the July 2, 1985 notice. EXPANDED MODIFIED MEDPAR FILE The Medicare Provider Analysis and Review (MEDPAR) file contains records for 100 percent of Medicare beneficiaries using hospital inpatient services. The records are stripped of most data elements that will permit identification of beneficiaries. The hospital is identified by the six-position Medicare bill number. The file is available to persons qualifying under the terms of the routine use act as outlined in the December 24, 1984, Federal Register, and amended by the July 2, 1985 notice.
